English Parent Consent to Participate Form
As a parent participant in the district’s parent involvement workshop sessions, you are
invited to participate in a research study which will be conducted by Gloria Hernandez, an
administrator with the district and a doctoral candidate in the Independent Doctorate, EDD
Department in the College of Education at California State University, Sacramento (CSUS). The
study will examine parent and teacher perceptions of the effectiveness of the district’s parent
involvement programs and workshop sessions.
You will be asked to complete two questionnaires about home-school communications.
Each questionnaire will take about 20-30 minutes to complete. Additionally you will be asked to
attend and participate in monthly parent workshop sessions where your input will be noted.
Finally, you will be invited to take part in a 30 minute interview at the completion of the study.
Some of the items in the questionnaire may seem personal, but you do not have to answer
any question if you don’t want to. Some of the interview questions may also seem personal; and
again, you don’t have to answer any question if you don’t want to.
There are a number of benefits of voluntarily participating in this research study. The
information you provide will help the district understand how we can be more effective in
working together with parents. We hope to learn how to better provide parents with information
and tools to increase students’ academic achievement and improve student learning.
All results obtained in this study will be confidential. Your responses on the
questionnaires will be anonymous. With your permission the interview will be taped. However
as soon as the tape is transcribed, the tape will be destroyed and you will be identified only as a
parent of a Hispanic, African American, or an English Learner student. Only group results for the
project will be reported.
You will not receive any compensation for participating in this study.

Consentimiento de padre/tutor legal de participar en un estudio de investigación
Usted esta invitado a participar en un estudio de investigación que esta siendo llevado a
cabo por Gloria Hernández, candidata a un doctorado en la facultad de educación en la
universidad del estado de California, Sacramento (CSUS), quien además es una administradora
del distrito. El estudio se enfocará en la efectividad del programa para participación de padres de
familia del distrito.
Se le pedirá llenar dos cuestionarios sobre el programa para la participación de padres del
que usted forma parte. Cada cuestionario tomara hasta 30 minutos para completarse.
Adicionalmente, usted pudiera tomar parte en la reunión de un grupo de enfoque junto con otros
padres y tutores legales, para hablar con respecto al programa para la participación de padres de
familia. La reunión del grupo de enfoque podría durar hasta 45 minutos.

Вы приглашаетесь для участия в опросе которое проводит Gloria Hernandez, аспирант
Педагогического колледжа в Калифорнийском государственном университете города Сакраменто
(CSUS) и администратор школьного округа. Этот опрос поможет определить эффективность
