Заполняя эту форму заявки, я хотел бы принять участие в Международном
Фестивале циркового искусства в г. Ижевске. С этой целью я сообщаю следующее:
With this form I would like to ask to be taken into consideration by the selecting
committee and have the possibility to partecipate to the International Circus Festival in
Izhevsk, Udmurtia, Russia. For this reason I declare what follows:

быть публично оглашена дирекцией фестиваля, к этой заявке приложена копия
моего паспорта, маленькая фотография (типа паспортной) и видео DVD .
I also declare that the information is true, I give the permission to it’s divulgence for the
production of the International Circus Festival. I enclose to this form, a photocopy of the
passport, a small photo (for documents) and a video DVD with my artistic performances.

Дополнения / NOTE:
* Форму следует полностью заполнить и послать по E-mail udmcircus@udm.ru,
по факсу 3412-78-78-96 (печатными буквами, чёрными чернилами) или почтой
426 057 Россия, Ижевск , ул. Красноармейская, 136, « Фестиваль » . Фото
в формате JPG или почтой вместе с видеоматериалом, что необходимо для
участия в Фестивале).
* The form has to be entirely filled in and e-mailed to udmcircus@rambler.ru or faxed to
+7-3412-787896 (in this case fill in the form in black letters or typewrite it). The photo, if
sent by e-mail, has to be in JPG format or otherwise it should be sent by ordinary mail (it is
necessary for the participation).
* Фестиваль имеет право вносить редакционные изменения. Участник своей
подписью соглашается с условиями и правилами участия, использования его
информации в рекламных целях без какой либо оплаты , также как и с фото и ТВ
съёмками во время репетиций и представлений .
* The Festival reserves itself the right to make editorial changes. The participants with
their signature accept the participation conditions and are aware that their photos and
texts could be introduced in the brochure or could be used to advertising purposes, without

payment, and that the rehearsals and the performances could be filmed by TV. The
Festival reserves itself the right on the photos and on the videos recorded during the
rehearsals and the performances.
* Фестиваль не несет ответственности за травматизм артистов или их болезнь во
время проведения Фестиваля также как и за порчу или утерю их реквизита
* The Festival isn’t responsible for possible artists’ incidents or diseases during their stay
in Latina or even for possible damage or thefts to artists’ or troupe’s costumes and
equipments
* Материалы, высланные для участия в Фестивале, возврату не подлежат
* If an artist candidate for the participation to the International Circus Festival will not be
selected by the committee, the material will not be returned.
* Заявка должна быть заполнена лично каждым артистом и участником труппы,
если информация будет не полной, Фестиваль будет вынужден отказать такому
участнику.
* The form must be filled in and signed by every artist and every member of a troupe who
would like to participate to the International Circus Festival . If the form isn’t complete or
if we lack the entire or most of the requested material, the candidate will not be considered
by the selecting committee and will be excluded from the competition.
* Во время пребывания в г. Ижевске артисты и их агенты (руководители) обязаны
соблюдать правила Фестиваля и временной распорядок во взаимодействии с
работниками Фестиваля.
* During their stay in Izhevsk , both the artists and the supervisors must respect the
Festival regulation and the different engagement times which will be communicated by the
staff.
